What is DBT Therapy and Who is it For?

For years, the clinical world used labels like "dramatic" or "borderline." But Dialectical Behavior Therapy (DBT) changed the game. It isn't just a therapy; it is a pedagogical manual for people whose "volume knob" for emotion is stuck at a 10.


The Biosocial Reality: You aren’t 'Dramatic,' You’re Wired Differently


At Think Well Live Well Counseling, we don't look at emotional intensity as a character flaw. We look at it as a biological transaction.

According to the Biosocial Theory, your struggle is the result of a "thin" emotional skin meeting an environment that didn't know how to hold it:


  1. The Biological Vulnerability: You were likely born with a hyper-sensitive amygdala (your brain's alarm system). You feel things faster, you feel them more intensely, and it takes your body significantly longer to return to "calm" than the average person.

  2. The Chronic Invalidation: You grew up being told to "just calm down" or "stop being so sensitive." When your internal reality is constantly denied, you stop trusting yourself. You begin to oscillate between shutting down and exploding, just to be heard.


The DBT Framework: Piloting the Ship During the Storm


DBT is the only modality that doesn't ask you to "stop feeling." Instead, it teaches you how to pilot the ship while the storm is happening.

The "Dialectic" is the heart of the work: It is the radical act of accepting that you are doing the absolute best you can with the tools you have, AND you must learn new tools to get the life you actually deserve.

  • Core Mindfulness: Moving from "Emotion Mind" (reaction) to Wise Mind (intuition + logic).

  • Distress Tolerance: Using TIPP skills (Temperature, Paced Breathing) to physically hack your body chemistry when you are too "hot" to think.

  • Emotion Regulation: Learning to label the "blob" of pain so it stops controlling your behavior.

  • Interpersonal Effectiveness: Using tools like DEARMAN, a framework that teaches you how to Describe the situation, Express your feelings, Assert your needs, and Reinforce boundaries while staying Mindful, Appearing confident, and Negotiating. It helps you set boundaries and ask for what you need without the soul-crushing guilt of self-abandonment.

Many of my clients in Indiana have tried "skills" before, but found that when the trigger is big enough, the skills vanish. This is because there is unprocessed trauma stuck in the basement of the nervous system.
